Explanation of Benefits 2012

You receive an Explanation of Benefits (EOB) from Community HealthFirst™ Medicare Advantage each time we process a claim. The EOB is not a bill. The information on the EOB helps you understand how your benefits are applied to that particular claim. It includes the name of the prescription filled, the amount you paid, the amount Community HealthFirst paid, and if applicable, other programs paid. It also tells you the amount credited towards your out of pocket costs.
