Best Available Evidence Policy

If you believe you have qualified for extra help and you believe that you are paying an incorrect copayment amount when you get your prescriptions at a pharmacy, our Plan has established a process that will allow you to either request assistance in obtaining evidence of your proper copayment level, or, if you already have the evidence, to provide this evidence to us. If you believe that you should be paying a different copayment amount, please call the Community HealthFirst Customer Service Department, 7 days a week, from 8:00 am to 8:00 pm, at 1-800-942-0247 (TTY Relay: Dial 7-1-1).
